The effect of rhetorical questions in this passage is to encourage the reader to reflect on some dilemmas, and come to his own conclusions. The author hopes his writing is persuasive enough that the reader will reach the same answers to these questions as the ones the author is proposing.
For example, the author asks whether Vietnamese people and the communists can be blamed for not trusting the United States. As evidence, he explains that the United States has caused a lot of violence and often pushed the Vietnamese to take up arms as self-defense. The author hopes that these questions will force the reader to put himself in the other person's shoes and understand the situation from the Vietnamese perspective.
The effect of the Rhetorical question creates doubt in the mind of the reader regarding the intentions of the United States of America. Stated, differently, the question begs the answer "Are we trying to help the people who are being oppressed if we take sides with their oppressors?"
Lines 203 - 214 also begs the question of how one expects to be trusted after we have maltreated them.
