It is a requirement before CVS Pharmacy will bill Medicare B for the
dispensed prescription. It will serve as an arrangement if patients requested that their health benefit payments be made directly to a designated
person or facility like pharmacy, hospital or physician. This form is called
Assignment of Benefits or AOB form.
