The last choice is the answer.
In my previous answer, I did state ONE use of the semicolon. This is another. The items you are trying to list all have commas in them. When you are trying to list items that have commas in them (for example, city and state), you use a semicolon instead of a comma to separate the items. Also, no punctuation is needed after the "are".
