I would choose the flu because that alone kills a lot of people, it kills most more people than most deadly diseases like Ebola.

I noticed that the other answer on here is a communicable disease rather than a non-communicable disease, so here is a non-communicable disease answer.

I would choose cancer because it is one of the leading causes of death each year. The cure that we have has lasting side effects, and most of the time it's non-effective. It can be incredibly painful and it poses a great threat to much of the world. It also uses up a lot of funds.
