Here's a whole list of reasons/excuses:
1. The goals were unrealistic.
2. There was no breakdown of how to achieve the goals.
3. Something unexpected (an emergency) came up.
4. There were more important things to do.
5. The goals were not specific/clear enough.
6. There were too many/too hard obstacles.
