You should first of all identify your points as x1 and y1 and x2 and y2. To get the slope, you take the difference of the ys divided by the difference of the xs. You then pick one of the two points and write the equation in point slope form. You then distribute so that you can clean it up and write it in slope intercept form. You should get y=3/4x+4
