According to the Frank-Starling law, the force or tension developed in a muscle fiber is proportional to the extent to which the fiber is stretched. In a clinical setting, increased blood flow into the heart (increasing preload) causes the heart walls to stretch. The Frank-Starling law of the heart relates preload to cardiac performance; a decrease in preload reduces the force of ventricular contraction and thus decreases stroke volume. As a result, decreasing preload generally leads to a decrease in cardiac output.

Increased preload increases stroke volume, while decreased preload decreases stroke volume by affecting the force of cardiac muscle contraction.
