Continuum of care consists of all levels of personal care as well as health care services available.
Long term care can be a part of the continuum of care, in the sense that these services can be provided to people at their home so that they could retain their independence and be cared for, from the comforts of the home, by their friends and family. The care can then be given in an informal manner.
Whereas other medical services can be given in a formal manner, by the different medical agencies, long term medical is given usually in an informal manner and meet both the medical as well as the non-medical needs of the people.
