etiquette: politely asking them to repeat it would be a good option, even if it feels a little bit awkward. (You could always say that "my apologies, I had trouble hearing your name, may you please repeat it? ...")

if you're writing a transcript, you can mark it:

[unintelligible 00:00:00]

(except make the "00" into the hour/minute/second)
