The point slope formula is,

[tex]y-y_0=m(x-x_0)[/tex]

Where,

[tex]m=\frac{y_1-y_0}{x_1-x_0}[/tex]

And,

[tex](x_1,y_1)=(-3,-4)[/tex]

[tex](x_0,y_0)=(0,2)[/tex]

So, 

[tex]y-(-4)=\frac{-4-2}{-3-0}(x-(-3))[/tex]

[tex]y+4=2(x+3)[/tex]
