Rudolph has remained drug free for two months minus a couple days when he used cannabis to settle his nerves by attending an intensive outpatient program in the Valley Rudolph had a sponsor and was an active member in a 12 step fellowship for his first month of his recovery, but has opted out to discontinue his participation in the fellowship Rudolph receives VA benefits, but does not want to go to the hospital for treatment his resistance are unclear. Rudolph is interested in receiving continued care for his drug and alcohol. Use Rudolph has been living with an army and his family has a burden. He helps around the house and is able to watch the kids to allow his friend and his friends wife to spend time together running errands and going out to dinner Rudolph smoke cigarettes and recognizes this is, not good for his health and also set a example for his friends children another reason why he wants to find another place to live. Rudolph continues to have minimal contact with his family who live locally Rudolph continues, attending the intensive outpatient program through without much success as he has been using cannabis and methamphetamine on a regular basis. His living situation is unstable still on the street near the west LA veterans administration he frequently attends the clinic appearing disshoveled Unaware of what day it is and appearing as though he has been in a fight Rudolph enjoy several cups of coffee while in the waiting area and seems appreciative of snacks available Rudolph has difficulty focusing on the group topics and appears to be distracted during either the individual or group sessions longest period of time Rudolph has been able to go without using methamphetamine is three days when asked how he was able to abstain for length of time his response is I cannot get to drugs to buy. My own task is to evaluate the severity of each patient that you have been assigned on each of the six dimensions of the ASAM criteria, using the information presented in a case scenario listed above what if any additional services will be required what level of care will you recommend for the individual? If currently in treatment will the patient remain in the same level of care down to a less intensive level of care or require a more intensive level of care what are the service needs relative to each of the dimensions? Is it to arrive at a severity level of 0-4 for each of the sixth dimensions, ensuring to document the rational for each dimension use the ASAM Level of care determination tool to arrive at the most appropriate least intensive level of care based on the patient’s risk level/ severity, level of functioning, and service needs
