In coding for skin lesions, code selection is based on the following:
a) Depth of the skin lesion and size
b) Greatest diameter of lesion, do not include margins
c) Greatest diameter of lesion and most narrow margins required
d) Greatest diameter of lesion and most narrow margins required multiplied by 4
