Ken Thomas (date of birth [DOB] 10/25/19XX) saw Jean Burke, nurse practitioner (NP), this past week. He was diagnosed with acute leukemia after several tests. You work with Ms. Burke, and you were involved with arranging Ken’s tests. Today, Ken’s adult child, Alex Thomas, calls you. Alex wants to know what is going on with Ken. You look at Ken’s health record and see that Alex is not on the disclosure authorization form. Per the facility’s policy, for information to be given to a patient’s family, a disclosure authorization form must be completed. Later Ken calls and asks why you did not update Alex on his condition. He sounds upset while he is talking with you.
