Since the 1999 Institute of Medicine Report, To Err is Human, patient safety in hospitals has:
a. Worsened according to an Agency for Healthcare Research and Quality report.
b. Improved so significantly that experts recommend focusing on other priorities.
c. Worsened which has led to federal laws that close hospitals which have poor safety records.
d. Improved markedly so that Joint Commission National Patient Safety Goals have been discontinued."
